Ok, it happened to me twice, once in 1998, the other time yesterday, in 2010. I typed crontab -r instead of crontab -e, and I hit enter. I'm such a fast typist: and I promptly deleted root's crontab on one Linux server. I had a backup, yes, sort of. But it's still embarassing. Why choose two letters so close on most keyboards? Why not even ask for confirmation when you are effectively about to delete an important file in /var/spool? Why not fixing a couple of lines of code in what? twenty years...
